Comfort Homesake is on a mission to increase national awareness about aging and health care planning choices until every family is prepared.

Advocacy Partnerships

Empathy is the cornerstone of the work we do. Comfort Homesake advocates for the elderly and ill through a number of compassionate and effective channels.

We create partnerships that help forward our mission and provide exposure and access to quality advanced care planning. Our current work includes the following partners:

ALAMEDA COUNTY HEALTH CARE SERVICES AGENCY: GETTING THE MOST OUT OF LIFE PROGRAM

Since 2012 Comfort Homesake has provided contract training and care coordination support to Alameda County's Getting the Most Out of Life Program. Comfort Homesake has conducted over 200 culturally relevant trainings and documented over 700 advanced health care conversations with people managing a chronic disease, who have a serious illness or are facing end of life. Throughout Alameda County Comfort Homesake shares the goal of reducing suffering and increasing quality of end of life care. We promote community based palliative care to reduce the stress of increased symptom burden, transitions from hospital to home and ultimately pain management that hospice care provides.

CALIFORNIA POLST COALITION

Comfort Homesake is a certified trainer for Physicians Order for Life Sustaining Treatment (POLST). POLST (Physician Orders for Life-Sustaining Treatment) is a form that states what kind of medical treatment patients want toward the end of their lives. Printed on bright pink paper, and signed by both a doctor/physician assistant and patient or medical agent, POLST helps give seriously ill patients more control over their end-of-life care.

INTERNATIONAL END OF LIFE DOULA ASSOCIATION

In 2016 Comfort Homesake dedicated staff received certified training in end-of-life doula and launched its No One Dies Alone Program. This program supports growth and development of the end-of-life profession through training and direct care for those who don't want to die alone.
Upon referral, Comfort Homesake supports "unbefriended" patients with a terminal illness by providing its No One Dies Alone vigil service at the bedside. ALAMEDA CONTRA-COSTA MEDICAL ASSOCIATION

Comfort Homesake is a community partner of the ACCMA delivering multi-cultural, multi-faith outreach for the Association through a contract with the TJ Long Foundation.

STANFORD UNIVERSITY SCHOOL OF MEDICINE

Comfort Homesake staff have been trained in Stanford University Ethnogeriatrics: iSAGE Program. Since 2016 staff participates in Stanford's "Letter to the Doctor" outreach plan.
