Comfort Homesake is here for you and your family as you navigate the difficult issues families must address when faced with the long-term illness or imminent death of a loved one. While you are  focusing on the work of living with an illness our staff is focused on you. We have assembled a team of professional researchers and caregivers who are trained to provide culturally competent services and resources.

Our commitment is to provide the highest quality of personal care, managed assistance for the patient, caregivers, family and friends.

Comfort Homesake works with you in the privacy of your home or office. We offer the following services to families through our programs that help caregivers be successful.

1) Become a confident resource for your family during a medical event.  
A chronic or terminal illness is devastating for the patient and everyone concerned. Hardest hit are family members who rely on the patient. We offer you and your family years of experience, providing the information you need to smoothly transition to living fully as your family goes through this transformational process. Services include family dynamic assessment and evaluation of the resources available.

2) Form a family-oriented plan for illness.
Even when we don’t expect illness or complications, pre-established plans are essential, as they  allow us to place your focus where it belongs during an emergency, on your family member. We offer assistance in developing key building blocks for you and your family, including:

  • Direct patient care
  • Medical directives
  • Family care giving plan
  • Preparation of financial resources
  • Community relations and support.

3) Form a  plan for disease prevention and/or management.
Keeping yourself healthy must be a priority – prevention is as vital as cure. Our staff will assist you in developing a Personal Commitment Plan for better personal health through prevention, disease maintenance and management. We will help you find the most current practices in managing your family member’s illness in a way that fits your life.

4) Promote nutritional wellness for your entire family.
Our personal diagnostics program will create a wellness strategy designed to fit you and your family. This plan addresses your family’s medical history as well as issues concerning you and your loved ones. A personal coach will advise and assist you with maintaining health and wellness through diet, exercise, and holistic care.

5) Caregiver training.
When disease strikes, it’s not just the patient who is affected, it is the whole family.  That means it is important for family members to learn caregiving skills. As most people living with serious illness want to stay at home, Comfort Homesake’s caregiver training helps you learn how to care for a loved one who is recovering from an illness or who is terminally ill.  

Additional support services provided are:

  • Direct patient care – culturally sensitive personal services.
  • Facility recommendations – when staying at home is not possible.
  • Family meeting facilitation – sometimes it helps to have a third party guide.
  • Caregiver training – helps calm the nerves and worries about caregiving.
  • End-of-life counseling – pure and simple support and comfort as needed.
  • Cultural competence – a commitment to observing traditions and beliefs.
